TEENAGER: Place your first, middle and last initials of your name if you consent to the statement below: I accept the opportunity to represent the Trinity Gardens Church of Christ Youth Choir, and agree to maintain the professional standards of leadership and conduct, as detailed by TGCOC.I agree to prepare for and attend all rehearsals (arriving promptly and remaining through the length of the rehearsal) of the choir of which I am a member. I will cooperate with my peers in building and maintaining the teamwork necessary to build a choir of the highest standards of musical accomplishment. If an unavoidable conflict arises I will notify TGCOC staff immediately. I understand my musical commitment to the entire season's schedule.

I understand the TGCOC maintains professional standards of leadership and conduct, and I support my child's commitment to abide by those standards. I will abide by TGCOC rules and regulations, and the schedule to be sent home in the registration package. I will record these dates in my family calendar.
